PIAA Physical Options

The Pennsylvania Interscholastic Athletic Association (PIAA) requires a Comprehensive Initial Pre-Participation Physical Examination (CIPPE) each year The CIPPE may not be authorized earlier than June 1st and shall be effective, regardless of when performed during a school year, until the next May 31st.

High School Athletes: The first four pages of the PIAA Physical can now be filled out online at www.familyid.com you will still need to hand in Sections 5 & 6 to the Athletic Trainer Jerry Dancho

Middle School Athletes: The first four pages of the PIAA Physical can now be filled out online at www.familyid.com you will still need to hand in Sections 5 & 6 to the Athletic Trainers.

Primary Care/Family Physician

You will need to schedule an appointment with your doctor and have them fill out the Physician"s portion of the PIAA form (Cost/deductibles vary).
